Dear Parents and Carers,

To ensure accurate record keeping, we are required to ensure the data we hold is correct each year. 

With this in mind, please find a link below to an online form which contains details of our Annual Permissions. Please could you kindly complete one form for each child who attends the school.

The online form replaces the previously used paper version, and removes the administrative burden on parents to print, complete and scan back to us.

If you had a younger child who was a new starter this year, you do not need to complete a form for them as the one submitted as part of their application is still valid. 

Please only complete if your child has been at the school for more than 12 months. 

We would be grateful if this could be completed by Friday 8^th November. We greatly appreciate your cooperation.
