If your child has a medical condition, such as asthma or an allergy, please make sure that you let us know. We can support children with their medical needs by sharing information. Forms to complete are available from the Letters & Forms page of the school website. The Asthma Allergy Plan and Allergy Action Plan forms can be found in the section at the bottom of the page called Medical Conditions and Allergies.
Forms are required to be completed annually in September so even if you completed one last year, we need an up-to-date form please.
Completed forms must be returned by Monday 12th September. Please ensure inhalers/epipens etc are in school as soon as possible in case they are required.
